Library Cards

TO OBTAIN A LIBRARY CARD:

Wisconsin Residents:

  • A free library card (see card picture at right) is available to any Wisconsin Resident.
  • However, if you would like a small key cardwith your free card, the cost is $1.00.
  • There is no minimum age requirement to get a card; parents may decide when to obtain a library card for their child.
  • Fill out an application at the Circulation Desk.
  • Present proof of current address (one of the following):
    • valid driver's license
    • utility bill
    • check book with name & address imprint
    • hunting or fishing license with address on it
    • envelope addressed to you with cancelled postage

NOTE: An applicant under 18 years old needs a parent/guardian signature on his/her application

Non-residents:

Non-residents may obtain a temporary card:

  • Fill out an application at the Circulation Desk
  • Present proof of address -- either the permanent address (if visiting) or the temporary address (if moving). Use one of the following:
    • valid driver's license
    • utility bill
    • check book with name & address imprint
    • envelope addressed to you with cancelled postage
  • Pay a $10.00 non-refundable fee

HOW LONG IS A LIBRARY CARD VALID?

Although you may use your passcard indefinitely, your record will "expire" annually. You will be asked to verify or update your personal information. The purpose of this is to keep the MORE database current. Patron records are removed from the computer system after 3 years of inactivity. To reactivate your borrowing privileges, you must present acceptable proof of address and apply for a new card. MAY OTHERS USE MY LIBRARY CARD?
Library borrowing privileges are non-transferable; each person wishing to check out materials must have his/her own library card.

MAY I CHECK OUT MATERIALS WITHOUT MY CARD?

In order to insure accuracy, it is strongly suggested that you present your library card whenever you wish to check out materials. As long as the computer system is up, the personnel at the Circulation Desk can look up your patron record. You will be required to show identification or answer personal data questions to prove who you are. NOTE: when the computer system is down (due to upgrades or equipment failure), you must have your library card with you in order to check out materials.

CHANGES TO PERSONAL INFORMATION

It is very important that you keep your patron record up to date. When your address changes, please let someone at the Circulation Desk know as soon as possible. You will be asked to show proof of your new address and to fill out a 'Change of Address' form. When your name changes, please fill out a new application at the Circulation Desk. You will be asked to show proof of identification and then you will receive a new library card (no fee).

LOST OR STOLEN CARDS

If your library card is lost or stolen, please ask for a replacement as soon as possible. Present proof of your current address at the Circulation Desk and pay the 25-cent card replacement fee. The lost / stolen card is de-activated and a new card is issued.
